1. Provide educational in-services and other educational opportunities to the community and providers in assigned territory

2. Work cross-functionally, with Admissions and Marketing Departments, to develop and carry out a marketing plan

3. Manage the marketing activities for an assigned coverage area, daily

4. Meet with appropriate parties to establish a positive referral relationship and to educate on referral appropriateness, including;

a. Physicians who influence or make referral decisions

b. Physician office nurses

c. Hospital discharge planners and floor nurses

d. Long-term care facility administrative and clinical staff

5. Develop opportunities to conduct in-service presentations for physician offices, hospitals, nursing homes, assisted living, Community Based Residential Facilities (CBRF) and other healthcare providers

6. Collaborate with Admission Department and clinical staff to manage intake of patient and families

7. Coordinate utilization review with providers

8. Meet or exceed Key Performance Indicators (KPIs), including;

a. Make 10 face to face contacts per day with referral sources

i. This could include multiple contacts within one facility, but must be face-to-face contacts with staff who have an impact on referrals - dropping off literature or telephone calls do not count towards daily goal

b. Visits at least six facilities per day

c. Develop business with month over month growth

d. Meet quarterly admission goals

i. Responsible for maintaining accurate daily, weekly, monthly and quarterly admissions numbers for their territory and quarterly plan

e. Maintain 100% daily documentation in Customer Relationship Management (CRM) system, including daily call schedules, weekly call schedules, track of all appointments and activities

9. Communicate, appropriately, with providers, attending physicians, other hospice staff members, and other agencies as needed to ensure excellent provider relations

10. Maintain confidentiality of sensitive information

11. Work closely with the hospice staff, Director-Clinical Operations and all Team Directors to insure that promised services are provided

12. Participate in hospice orientation and ongoing education programs

13. Participate in committees and on special projects, as assigned

14. Demonstrate proficiency and willingness with sign-ons and information sessions, as assigned

15. Demonstrate 100% compliance with all reporting processes

16. Develop and implement a(n);

a. Quarterly marketing plan, submitted two weeks prior to the next quarter

b. Weekly sales call plan that follows quarterly market plan and goals

c. Daily call plan with at least ten (10) sales calls per day that align with quarterly market plan and goals

d. Appropriate pre-call plan for each sales call

e. Post-call follow-up for each sales call made, in collaboration with supervisor

17. Adhere to monthly community education budgets as set by program site leadership

18. Document customer service issues on the Adverse Event Form according to protocol

19. Complete and maintain all HCC competencies

20. Performs other duties as assigned including those within the scope of current, valid professional licensure/credentials (i.e. RN, SW, Chaplain, MT, HA, etc.) Employees should reference the position specific job description for additional details on the essential functions of the clinical discipline

21. Other duties as assigned


1. Bachelor's degree, preferred -or- 5+ years' experience in hospice, home health, healthcare sales or marketing, in lieu of degree

2. Excellent written and verbal communication skills

3. Demonstrated ability to work closely with a variety of stakeholders

4. Effective problem-solving skills

5. Understanding of referral patterns

6. Experience with, or willingness to learn, Medicare regulations and related reimbursement practices related to hospice and long-term care

7. Proficiency with Microsoft Office Suite products

8. Available for after hours and weekend referrals

9. Demonstrate familiarity with policies of the agency and state(s) and federal regulations pertaining to hospice services

10. Ability to work as part of an interdisciplinary team
