Description 

A great opportunity to work within a role supporting others in a community environment. We are looking for a dynamic individual who is passionate in empowering individuals with greater independence and improved wellbeing.
The role will require you to provide a range of social, emotional and practical support for people with Mental Health and associated problems who are homeless or at risk of homelessness and would require short term support to access and maintain suitable accommodation, and mental health services.
The role is based across Neath & Port Talbot but the office base will be in Swansea.
The support will be primarily Monday to Friday between 9am - 5pm although there is scope for flexible working. The role will involve a large amount of lone working, and working part of a team.
We are looking for an individual who is passionate about helping the most vulnerable adults in Neath & Port Talbot area. Has a good understanding of housing and homelessness legislation, and the benefits system.
Experience
Experience of working in the health and social care field, providing support, assistance and advice to individuals experiencing mental health and associated problems
Experience of working with people to enhance and promote independent living skills.
Experience of providing support to people living in supported accommodation and/or the community.
Qualifications and skills
Qualification relevant to the work - i.e. QCF Level 3 Health and Social Care
Full current driving licence and access to your own transport
Ability to communicate in Welsh is desirable BUT NOT ESSENTIAL
Excellent verbal, written and IT skills, including basic word processing, and inputting information
